Polymyalgia Rheumatica: "Corticosteroids, Rise and Walk"
Abstract
Polymyalgia rheumatica (PMR) is an infl ammatory condition that primarily affects individuals over the age of 50 and is characterized by pain and stiffness in the shoulder and pelvic girdles. We present the case of a 72-year-old man who experienced several weeks of progressive pain in the hips and shoulders, with signifi cant functional limitation. Initial suspicion of statin-induced myopathy led to discontinuation of lipid-lowering therapy, without clinical improvement. Laboratory tests revealed elevated infl ammatory markers, particularly C-reactive protein. A diagnosis of PMR was made, and treatment with prednisone 15 mg daily led to rapid symptom resolution. A progressive tapering schedule was implemented over six months, with calcium and vitamin D supplementation to prevent corticosteroid-induced osteoporosis. This case highlights the importance of early recognition and individualized treatment in primary care to ensure symptom control and reduce the risk of complications or relapse.
